Managing diabetes often involves much more than monitoring blood glucose. Meals, medication routines, physical activity, foot care and changes in general health can all play an important role in maintaining wellbeing.
We provide practical in-home support tailored to your individual diabetes care plan and level of independence. Depending on your needs, our support workers and nursing team can assist with everyday routines while working alongside your GP and other health professionals. We also help identify and communicate changes in your wellbeing so that concerns can be addressed by the appropriate member of your healthcare team.

How To Fund Your Diabetes Management at Home
You can fund diabetes management care services from EnableU using:
- Support at Home (SAH) / Home Care Packages (HCP): If you are on aged care funding, services can be funded directly from your support at home package.
- NDIS (National Disability Insurance Scheme): Participants of the NDIS can utilise funding from their core or capacity building budgets to access services.
- Private Funding: Individuals who don’t qualify for government-funded care or prefer more flexibility and choices can choose to privately fund their services.
Currently we do not support individuals in Residential Aged Care or those on CHSP funding.
