What is Short Term Restorative Care?

Goal-Focused Support Delivered by an Experienced Clinical Team

The Restorative Care Pathway is a short-term pathway under the Australian Government’s Support at Home program. It replaced the former Short-Term Restorative Care Programme when Support at Home commenced on 1 November 2025.

Rather than simply doing tasks for you, restorative care focuses on helping you maintain, rebuild or relearn the skills needed to do more for yourself.

You can receive up to 16 weeks of coordinated restorative care, primarily involving allied health and nursing professionals. Your care is delivered by a multidisciplinary team based on the needs identified in your aged care assessment and the goals you want to achieve.

The emphasis is on working with you, rather than creating unnecessary dependence on long-term support.

How Is Restorative Care Funded?

One of the important benefits of the Restorative Care Pathway is that its funding is separate from your regular ongoing Support at Home funding.

Each restorative care episode provides approximately $6,000 in dedicated funding for up to 16 weeks of intensive services.

Where there is evidence that additional funding is required to achieve your assessed restorative goals, eligible participants may receive another funding unit, providing up to approximately $12,000 for the episode.

This means that if you already receive ongoing Support at Home services, you can access the Restorative Care Pathway alongside those services rather than having to use your normal quarterly budget to pay for the restorative episode.

Participants may be eligible for up to two non-consecutive restorative care episodes within a 12-month period, with at least three months between episodes

Yes.

The Restorative Care Pathway can be accessed alongside your existing ongoing Support at Home services if you’ve been assessed and approved for restorative care.

For example, you might continue receiving your normal cleaning or personal care services while also accessing intensive physiotherapy and occupational therapy through your restorative care funding.

The two funding streams are managed separately.

No. You can be assessed and approved for the Restorative Care Pathway independently, even if you are not already receiving ongoing Support at Home funding or services.

The pathway can be used alongside or instead of ongoing Support at Home services. If approved, you receive separate short-term restorative care funding for your episode, rather than needing an existing Support at Home classification first.

You still need to complete an aged care assessment and be found eligible for the Restorative Care Pathway. If your needs change during or after the episode, your care team can also help identify whether you may need ongoing Support at Home services afterwards.

Eligibility is determined through an aged care assessment.

Your assessor considers whether short-term, intensive clinical intervention is likely to help you regain or maintain function and whether you’re willing and able to actively work towards restorative goals.

If you’re already receiving Support at Home and your circumstances have changed, a Support Plan Review may be used to consider whether the Restorative Care Pathway is appropriate.

It depends on the type of service you are receiving.

Clinical Supports, including nursing and physiotherapy, have no participant contribution and are fully funded by the Australian Government.

If your restorative care plan includes Independence or Everyday Living services, you may need to contribute towards those services according to your assessed contribution rate.

As your restorative care provider, we will explain any applicable contributions before services are delivered.

Yes, where you’ve been assessed as needing them.

Participants in the Restorative Care Pathway can also access approved Assistive Technology and Home Modifications funding to support their restorative goals.

For example, this might include suitable mobility equipment or changes that improve safety and accessibility at home.

The funding is provided through the separate AT-HM Scheme rather than being taken from your restorative care budget.

Your provider should start thinking about this from the beginning of your restorative care episode.

At the end of the pathway, your restorative care partner and clinical team prepare an exit plan outlining your progress, remaining needs and recommendations.

If you need ongoing or higher levels of support, a Support Plan Review can be requested to consider changes to your ongoing Support at Home services or funding.

It serves a similar purpose, but the former Short-Term Restorative Care (STRC) Programme was replaced by the Restorative Care Pathway when Support at Home commenced on 1 November 2025.

The new pathway sits within Support at Home and provides up to 16 weeks of goal-focused, multidisciplinary restorative care.
